We have a five digit home phone number. Put it into a six box field ... Computer says "No". Can't argue, just put a zero on the end.

It's more the case that
A) software devs often assume these problems are already solved by the libraries/packages they use to shortcut the development process, because they don't have the time/budget to write raw code each time. (This is assuming the company involved is not just buying in a 3rd party SaaS system and branding it)
B) Edge cases are weird, always have been.
C) ASCII isn't not enough anymore, with non english names needing to be supported. If you're not fully supporting utf8-mb4 then you'll lose characters.
D) lots of packages come with default configs that don't match up, and if you have a non utf8 setting for the encoding anywhere, (from network settings, server host, web server software, code libraries, db settings, etc etc), then bang goes any proper character handling.
E) Most devs now are specialists in one area, not responsible for the entire stack.

None of this in insurmountable, but it all boils down to a complete lack of caring from upper management. They just want it built fast, and ignore problems.

My daughter has a Welsh name which includes an a with a circumflex - â. She lives in England and it has caused her some trouble in the past but no major problems, it often gets input as a plain lower case a. The biggest issue is people not pronouncing it correctly

A couple of times I was involved with the procurement of software packages (health and energy) and USA products were in the frame. Even something as simple as 'Title' (Mrs, Mrs, Sir, Baron etc) generated a raft of change requests never mind names with ^ or ' + umlauts, cedillas and hyphens. More often than not workarounds rather than changes were pushed at us. 'Brigadier-General doesn't fit in the 'Title' field? Call him BrigGen' sort if thing. All the packages resisted changes and having to get into version management and, if you were a big beast like SAP it was 'here's the product' and that was that.

Yup. And the less done "in house", meaning more 3rd party libraries/packages/software brought in: the more there is of that limitation of "catering only to the lowest common denominator"

The last firewall update our supplier did locked us out of our network for over 3 hours. Luckily nobody called us for a life saving event. The idiots did both the online server and the hot standby at the same time.

Well they didn't pluck the number out of thin air or invent it.
