Where can I find a relay base/ box / holder

I need to replace a relay box/base/holder (what ever they are called). It's for a 5 terminal relay, and it needs to be fused. I know they exist, because I have one that needs replacing. I'll be buggered if I can find one though!

Just stick a in lune fuse in the wire that is already fused.
