I probably know more than most people about telecommunications manholes, but I worked for BT for 10 years and knowing about manholes was a part of my job for them.

I used to get cast aircraft parts made by a firm who's side gig was making replica manhole covers.
They had a good old boy pattern maker, he'd take rusty knackered old covers and make a new pattern of them in wood, filling in blanks etc as required. They had a pretty steady trade in making one and two offs for listed buildings and the like.

I too worked for BT, and have seen a few. One in particular comes to mind, you looked at the tide times before working there. Near high tide it took at least 3 pumps to drain it, amd at least 2 to keep the water down.
If you had a big cable open it was a bit bum-clenching until it was closed again, and if you were down there and the pumps stopped it was every man for himself, with the last man liable to get damp around the edges.
