The door doesn't need to be super close fitting does it?
So you could just have a 'door' on legs and just push it up against.

No it doesn't, that was plan A which was to get a metal plate which just rested against the hole at an angle so there was still room for air to get it. Might work fine for that door too I guess.

Weld up a frame and bolt it to / through the flagstones. Bolt the door to that. You could seal around edges with fire cement / render etc to cover it.
