Releasing its report into the incident today, the police watchdog said it was "astonishing" that procedures were not in place to prevent such an accident happening in the classroom.
Micklethwaite loaded live ammunition into a .44 Magnum revolver, failing to check whether it was a dummy round after taking it from a Quality Street tin used for storage.
He told the IPCC he believed all ammunition in there was inert, adding that he "wouldn't expect to find them (live ammunition) in a flipping Quality Street tin, just chucked in there loosely with all the other bits and pieces." He added: "It's just a no no."

Incredibly, from the gist of this story it looks like they're going after the supplier, like it's his fault that an armourer can't tell the difference between live and blank rounds. If a stray live round found its way into a box of blanks then it's a big problem, but it still hasn't killed anyone until an armourer loads it into a gun. These things might happen with recruits, a stray round left in a pocket accidentally gets loaded along with blanks in the field by a tired recruit with limited experience, but there is no excuse for a professional armourer doing it.

Some do - I've only ever seen one - it had a red plastic section instead of a bullet, that had 'burst' when used.
But I think it was said that they were also using lookalike fakes on set too.
