When the bloke at the top shoots someone 'byaccident' there is no excuse.
Ultimately he hired the people and approved the process. So the buck stops.

Well there is, for all the reasons outlined. The emotional response is to throw the book at whoever you can but if you've done due diligence then the penalty is somewhat reduced.

Mind you, it's probably worth a try next time you're up in front of a magistrate.

I know this is rocket science, but exactly who requested that live ammunition be carried onto the set, and who carried it onto the set? If it was not requested and was unknowingly transported onto the set, exactly who was responsible for live ammunition to be carried onto the set?

I do tend to look at these sorts of news reports from a much simpler perspective. I might for example imagine that I am there, on this closed set, miles from anywhere and then I wonder who on that crew would NOT know when some of that group are having fun shooting tin cans.

I may be wrong of course but how could anyone miss a '45 being let off anywhere near them. You would probably have to literally be miles away so my gut instinct tells me it was an open secret. Everybody knew, nobody stopped it. There were hundreds of live rounds recovered from this set after the fact.

The truth will out and I expect that to include those who were "playing" with live rounds on a film set. It is perfectly possible to establish forensically who has and hasn't fired a hand gun. If those examinations were not made, that would be a political decision.

If everybody "knew" then everybody is culpable to a greater or lesser degree to be determined by whatever evidence is eventually revealed.

I've only seen the target shooting mentioned on here just after the shooting happened, is there any truth in it or did someone just make that up on the interweb?

I'm not an expert, but if I was gonna 'shoot' somebody, I'd shoot it at the floor first, just to be safe.

Gun safety is built around the Swiss cheese model.

Keep live ammo away from guns unless you are preparing to shoot in a safe place
Treat guns as loaded even when they are not.
Don't point any gun at people unless you intend to shoot them
Make sure the gun is safe before you pick it up/accept it from someone.
Use all the safety features (safety catch, half cock notch, etc), but don't rely on them, particularly on old guns or target guns.
Don't put your finger on the trigger until your sights are lined up and you are ready to shoot.

All this stuff is Firearms 101, and any competent armourer/weapons master on a film set would insist on it. We are left with one of three possible options.

Alec hired an incompetent armourer.
Alec ignored a competent armuorer.
Alec intended to shoot someone.

Like I said, Swiss cheese. A single mistake (say allowing live rounds to get onto set) would not have caused a tragedy. It would take multiple errors to do that.

If you hire a brain surgeon you would require some proof of competency. Same with people in charge of guns.

Great. I know nothing of guns, so would probably put my trust in the people who provided it and those asking me (directing) what to do with it.

But brain surgery, that's an interesting comparison. AFAIK you don't have any medical qualifications or professional experience, so how would you as a potential innocent victim / patient (delete as applicable) check one out before going under the knife?

I ask having met four neurologists and had brain surgery recently, and now wonder what and how you think I should have checked?

According to the local sheriffs office, 500 live rounds were recovered from the scene.

Also, she was not present and really, not being present ought to mean no guns on set! Smacks of tokenism as in, yes they hired an armorer but that in reality she had no authority to perform her role.
